1.入会申し込み書、銀行振り替え依頼書(要記入捺印) 
 ※18歳未満の場合、保護者の署名・捺印が必要となります。
2.入会金・年会費
3.月会費2ヶ月分
※16日以降入会の方は1.5ヶ月分
4.引き落とし予定の金融機関のキャッシュカード、銀行印。
5.身分証明書(免許証、保険証など)

1. Completed Membership application form, Bank transfer request form (with signature/stamp ‘hanko’) *To be signed by parent/guardian if the participant is under 18 years of age.
2. Activation fee and Annual fee.
3. 2 months membership fee in advance
* 1.5-months membership fee in advance if joining after the 16th
4. Debit card for auto-payment and ‘hanko’.
5. ID (Drivers’ license, insurance, etc)

費用

年会費　3600円/1人/年(全会員種別、複数種別会員の場合も同)
1月の月会費(12月27日引落し)とともに年会費3.600円を引落しさせていただきます。
入会時は300円×12月までの月数。
休会や退会による返金は致しませんので予めご了承ください。

Annual fee:  ￥3,600 per individual (applies to all types, multiple membership holders).
The annual fee of ￥3,600 is billed and withdrawn from the bank account with January’s monthly fee (on December 27). 
Annual fee for partial year: ￥300 x number of months.
No refund if you freeze or cancel your membership early.

・柔術の回数制限会員は毎月制限回数までクラスに参加出来ます。未消化分の持ち越しや返金は出来ません。制限回数以上に参加する場合には、1クラスあたり男性1.800円、女性1.200円の都度払いとなります。

BJJ limited membership allows you to participate in classes up to the specified number of times each month. There is no carryover or refund for any unused classes. If you want to attend more classes than your limit, you willl need to pay a per-class fee of ￥1,800 for men and ￥1,200 for women.

柔術キッズ・ジュニア（6〜15歳・中学生まで）

・キッズ会員は曜日固定。休みによる振り替えは出来ません。

Kid’s BJJ membership is on a set schedule. We are NOT able to offer makeup classes.

・ジュニア(中学生)はインストラクターとの話し合いで、キッズクラスか大人の柔術クラスのどちらかに参加出来ます。大人の柔術クラスに参加する場合には、週1会員は月5回まで、週2会員は月10回まで週3会員は月15回まで曜日を問わずクラスに参加出来ます。

Junior BJJ.  Choose Kid’s BJJ or Adult’s BJJ after discussing with an instructor. For Adult BJJ, choose to participate once per week (up to 5 times a month) or twice per week (up to 10 times a month) or three times per week (up to 15 times a month). 

レディースキック（中学生・13歳以上）

・レディースキック回数制限会員は制限回数までクラスに参加可能です。未消化分の持ち越しや返金は出来ません。制限回数以上の参加は1クラス1,200円の都度払いとなります。

月会費の現金払いについて

外国人の方など月会費の支払いが、銀行口座振り替え出来ない、もしくは希望しない方は現金払いも対応致します。
但し、3か月分ごとのまとめ払いが基本で途中退会など、理由を問わず返金は出来ません。
2か月ごとの場合10%、1か月ごとの場合20%が割り増しされます。同じく返金は出来ません。入会時にご相談ください。

For membership and lesson fees, we accept cash (to include Japanese bank account direct withdrawal).  Fees are charged 3 months at a time.  There is no refund regardless of the reason for terminating membership.

You may pay for less than 3 months at a time for an additional fee.
For 2 months, membership fee + 10%
For 1 month, membership fee  + 20%

家族割り引き

同一口座より引き落とし手続きをしている家族(夫婦、親子、兄弟)は2人目以降が1人1.100円引きとなります。

休会・退会・各種変更手続きについて

休会される場合は、1ヶ月につき2,200円の手数料がかかり、退会は未納月謝が清算されたことを確認したのちに受理させていただきます。

いずれの手続きも直接ジムにて前月5日までに手続きを完了させて下さい。

(例)
4/1に退会(3/31まで会員)の場合、4/1から休会する場合、4/1から月5回会員から 
無制限会員に変更の場合など3/5までに手続きが必要。
尚休会中に月会費の変更があった場合、復会後は新月会費が適応されます。

入会キャンペーンのご利用に関して

当ジムが入会キャンペーンを行なっている期間中、以下の決まりに沿って入会手続きを完了された場合にャンペーンのご利用が可能です。

月会費の支払いが銀行振替で、当ジム初回入会者の方が対象となります。会員種別は問いません。月会費発生より６ヶ月間は退会出来ません。途中退会する場合には残りの会費を精算後の退会手続きとなります。

無料体験について

柔術、キッズ柔術、レディースキックの各クラスを未経験者お1人様、適応クラス1回に限りまして体験入会が出来ます。
道着は無料レンタル可能です。トレーニング中の強制などはありませんのでお気軽にお申し込みください。
